Acupuncture for Back Pain

Evidence-based care by nationally licensed practitioners — not a spa certificate

Sudden back pain during your trip — or chronic pain you've been living with?

Whether you woke up in your hotel with a locked back, stepped off a long flight stiff and aching, or have been carrying chronic low back pain for months — Japanese acupuncture is a practical option. And the standard of care is higher than most visitors expect.

🏛️ In Japan, acupuncture is a nationally licensed profession. Practitioners must pass the same national exam system that licenses doctors, dentists, and nurses. Our clinic's acupuncturists hold these Harikyu-shi (針灸師) national licenses. This is very different from massage or wellness-spa services, which carry no such requirement.

At Nagaoka Clinic, our licensed acupuncturists work alongside director Taro Kawano (nationally licensed judo-therapist / manual therapist, 14+ years of practice) to build a plan around your specific type of back pain.

3 reasons acupuncture works for back pain

① Reaching deep muscles that hands cannot

The two muscles most often driving chronic low back pain — the multifidus (spinal stabilizer) and the iliopsoas (hip flexor, chronically shortened by long sitting or flying) — sit too deep for surface massage to reach effectively. An acupuncture needle (0.16–0.25 mm, no cutting edge) travels through skin and superficial tissue to reach these muscles directly, releasing tension and restoring local blood flow where hands simply cannot go.

② Triggering the body's own pain-relief chemistry

Needle stimulation prompts the central nervous system to release endorphins, serotonin, and noradrenaline — the body's own pain modulators. This explains the common experience of deep heaviness during needling followed by a noticeable lightening of pain. In chronic pain, the nervous system itself becomes sensitised (central sensitisation); this neurological pathway is part of why acupuncture addresses persistent low back pain differently from painkillers.

③ Improving local circulation and calming the autonomic nervous system

Chronic back pain is often associated with reduced microcirculation in the affected area. Acupuncture promotes vasodilation and improves blood flow to oxygen-starved tissue. It also shifts the autonomic nervous system toward the parasympathetic (rest-and-recover) state — particularly relevant for back pain that worsens under stress or in cold weather.

Which type of back pain responds best?

🕰️
Chronic ache
Dull heaviness lasting 3+ months. Deep muscle tension and reduced circulation.
✈️
Travel / desk stiffness
Worsens after long flights or sitting. Iliopsoas and multifidus over-tension.
Radiating pain
Pain spreading to buttocks or leg. Piriformis syndrome or disc-related nerve irritation.
🔍
"Scan showed nothing"
No structural finding on MRI. Non-specific LBP (~85% of all back pain). Functional causes.

If your pain is primarily from structural issues (significant disc herniation, scoliosis, fracture), we will assess this at the first visit and advise whether manual therapy, acupuncture, or a hospital referral is the more appropriate path.

⚠️ Please visit a hospital first if your back pain is accompanied by: rest pain that keeps worsening / fever / changes in bladder or bowel function / weakness or numbness in both legs / unexplained weight loss. These may indicate conditions requiring imaging.

How a session works at our clinic

1
Consultation (English + translation app)
We ask about location, onset, triggers, and nature of pain using simple English plus translation apps. No Japanese required from you.
2
Movement and palpation assessment
We check range of motion and identify tight muscle groups and tender points (trigger points) to map where the needles go.
3
Acupuncture treatment
Sterile, single-use needles are placed at target muscles — commonly multifidus, iliopsoas, piriformis, and erector spinae. Low-frequency electrical stimulation (electro-acupuncture) may be added to deepen the effect. All needles are disposed of after use.
4
Highvolt electrotherapy (if appropriate)
For acute pain or nerve-related symptoms, high-voltage pulse therapy targets inflammation around nerve tissue.
5
Self-care advice
Simple stretches and posture tips demonstrated by gesture and diagram — language is not a barrier here.

Pain Relief Session ≈ 60 min. Premium Acupuncture Experience ≈ 90 min. See plans below.

Clinical evidence

Acupuncture for low back pain is among the most researched topics in complementary medicine. Here are the key studies behind our approach.

① Vickers et al. — Arch Intern Med, 2012 · PMID: 22965186

Individual patient data meta-analysis of 29 RCTs, ~18,000 patients with chronic pain (back/neck pain, headache, osteoarthritis). Acupuncture produced statistically significantly better outcomes than both sham acupuncture and standard care for chronic low back and neck pain (p<0.001). The largest individual-patient-data analysis in the acupuncture field.

② WHO — Acupuncture: Review and Analysis of Controlled Clinical Trials, 2002

The World Health Organization classified low back pain (LBP) as one of the conditions for which acupuncture has been shown through controlled trials to be effective.

③ Japanese Ministry of Health, Labour and Welfare — National Survey 2022

Back pain is the #1 complaint among Japanese men and #2 among women (~91–114 per 1,000 population). Approximately 85% of cases show no structural finding on imaging ("non-specific LBP"), making functional approaches like acupuncture a primary treatment option.

We aim to support your recovery and relieve pain. Individual results vary. This article is not a promise of cure. If you have severe or rapidly worsening symptoms, please see a hospital first.

FAQ

Does acupuncture hurt?
Japanese needles are extremely thin (about a hair's width, 0.16–0.25 mm) with no cutting edge — very different from injection needles. Most people describe a sensation of warmth or deep pressure ("deqi") during needling. Strong pain is uncommon. We check your comfort and adjust stimulation throughout the session.
I have a herniated disc / sciatica. Can acupuncture help?
Disc-related and sciatic-type pain are among the conditions we see regularly. For pain radiating into the leg, we often combine acupuncture with highvolt electrotherapy targeting the nerve pathway. We cannot promise results in advance, but we give you an honest assessment at the first visit.
I only have one day in Fukuoka. Is a single session worthwhile?
For many acute conditions — travel fatigue, sudden stiffness from a long flight — a single Pain Relief Session often provides noticeable relief. Chronic conditions benefit more from multiple visits, but we will be upfront about this so you can decide before the session begins.
How do you handle the language barrier?
We use simple English and translation apps (Google Translate, ChatGPT). For booking and detailed questions, LINE text in English is the fastest — we reply in English. Email also works well.
I take pain medication. Is acupuncture safe to combine?
Acupuncture does not conflict with common pain medications. Please let us know what you are taking at the start of your session. If you are on blood thinners (warfarin, etc.), inform us so we can adjust needle depth and placement. Do not change prescribed medication without consulting your prescribing doctor.
Where is the clinic and how do I get there?
